If students need to replace any lost or damaged items, please complete and submit the appropriate application form.

Replacement of Lost Student ID Card:

Students must bring their ID cards to school and wear them around their necks. In the case of a lost student ID card, please obtain a replacement card form from the office. Submit the completed form to the Student Affairs Office, together with the appropriate fee. The new student ID will be issued approximately 14 working days after submission.

Replacement of Student Pick-up Card:

Parents or appointees have to produce the student pick-up card when picking up students. If parents have lost the student pick-up card or need to apply for a replacement, please submit a written request with the processing fee to the Student Affairs Office. It requires 2 working days to issue a new card. Parents who fail to produce the student pick-up card should inform the Student Affairs Office. The parent’s ID card will be checked for verification. If the school is in doubt, it has the right not to release the student to the party concerned.

Re-ordering Learning Materials:

All school-based teaching materials are prepared to enhance and complement classroom lessons for effective learning. Therefore, if students lose any such materials, they need to replace them immediately. This is to ensure that their learning process is not interrupted or affected. It takes at least 3 working days to replace school materials.
