Notes On Completing of The Application Form
- 你在此表格所提供的個人資料，將用於學位分配事宜及其他與教育有關的用途。獲得「自行分配學位」的申請人姓名將在有關學校張貼。
The personal data contained in this form will be used for student placement and other education-related purposes. Names of successful applicants for the discretionary places will be posted at the schools concerned.
- 本校可能將收集所得的個人資料向其他政府部門或你的子女所轉讀的學校披露，以便核實資料及作其他與教育有關的用途。
The personal data collected may also be disclosed to other Government departments and schools to which your child is allocated/transferred, for verification of information and other education-related purposes.
- 你必須在此表格提供所需的個人資料。倘若所提供的資料不充足，本校可能無法辦理有關申請。
You should provide all necessary personal data in this form. Otherwise, your application cannot be processed due to insufficient information.
- 根據《個人資料(私隱)條例》第18和22條及附表1第6原則的規定，你有權更改或查詢所提供的個人資料。
You have the right to access and update personal data in this form, subject to Sections 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance.

Please submit the completed application form in person or by registered mail with the following:
- 貼上$1.40郵票之長度回郵信封3個(信封面請註明學生姓名及地址)
3 stamped self-addressed envelopes
- 申請人之香港身份證明書或居留證明副本乙張
Copies of identification document(s) or proof for legitimate residing in Hong Kong
- 申請人須遞交最近一年之學期成績表或評核証明影印本
Copies of report cards of the most recent year
- 申請人須遞交最近半年之學生手冊影印本
Copies of student’s handbook of the last 6 months.

有關入學之申請，獲邀參加面見者需於面見日繳交五十元面見費。
A non-refundable and non-transferable application fee of HK$50 will be charged on the day of interview.
